Employers, healthcare
organizations and government officials from across the Greater Louisville area
are exploring the development of a new initiative called a Community Health
Record Bank. When built, the Community Health Record Bank (CHR Bank) will store
and protect private health information for individuals and their families.
Individuals will then be able to make their information available to their
health professionals through a secure electronic network. The CHR bank is
expected to lead to improved
quality of care, better patient safety and lower costs of healthcare for
individuals. The CHR bank may also offer
new benefits to public health, such as improved health status measurements
through new information tools, improved quality of healthcare services through
better information, improved health education through targeted education, and
improved services for low income and un and under-insured populations.
The following diagram shows how the
CHR Bank may work.

Keeping this information in mind, when
you are ready, please
press continue and answer the
questions provided. Your responses will be used by the Louisville Health Information
Exchange, Inc., a not-for-profit community organization, to design the CHR Bank to be as beneficial as possible for your group or
organization and those you serve.
