This survey is for people representing hospitals and other in-patient providers serving patients in the greater Louisville area.   If you represent one of these organizations in any of the following roles, please continue:

  • Senior Leadership

  • CIO, CTO, CMO, CMIO

  • Department Leadership (e.g. ED, Radiology, Lab, etc.)

  • Professional Leadership (e.g. nursing, physicians, etc.)

  • Board of Directors
